    • Provide recommendations and adaptations to the client's environment to facilitate accessibility and functional independence.

    • Assess the risk of falls and implement preventive measures to enhance safety.
    • Educate clients and caregivers on strategies to minimise fall risks and improve balance and coordination.
  9. Education and Self-Management:
    • Educate clients and their support network on their condition, treatment techniques, and self-management strategies.
    • Provide guidance on maintaining physical health, preventing injuries, and managing symptoms effectively.
  10. Progress Monitoring and Evaluation:
    • Regularly assess and review the client's progress towards their functional goals.
    • Modify treatment plans as needed and provide feedback to the client and their support network.
  11. Collaboration and Advocacy:
    • Collaborate with other healthcare professionals, such as doctors, occupational therapists, or speech therapists, to coordinate comprehensive care.
    • Advocate for the client's needs, ensuring appropriate supports and resources are available through the NDIS.
  12. Documentation and Reporting:
    • Maintain accurate and up-to-date records of assessments, treatment plans, progress notes, and any necessary reports.
